Our Assessments

At Vive, our mission is to provide individualized, holistic care that addresses the complexity of your pain and fatigue. Our detailed assessment process is the first crucial step toward developing a personalized treatment plan that works for you.

What’s included in our assessments?

  • A comprehensive, whole-of-person assessment, undertaken over two extended consultations.

  • A third consultation dedicated to discussing your tailored management plan and ongoing treatment

  • A summary report to share with your health team, supporting any further care or evaluation required

How does it work?

Access Our Patient Portal & Complete Your Registration Form

Register for our online patient portal here. After this, you’ll receive a welcome email with key information, and a link to complete our online registration form.

Book Your Free Intake Meeting

In your welcome email, we’ll also invite you to book in for your free intake meeting with one of our clinical team members. This is a short 10-15 minute meeting, in which we will chat with you to find out more about your circumstances, symptoms and needs. You’ll have the opportunity to ask any questions you might have, we’ll make sure we have everything we need to book you in, and we’ll discuss who on our team is likely to be the best fit for your needs.

Book Your Assessment Sessions

After your intake meeting, we’ll confirm with you that we have everything we need and invite you to book in for your three assessment sessions (either directly through our admin team, or via our online booking system on the Patient Portal). These are ideally spaced a week apart (or less) and recommend against more than a week and a half or so between bookings, as this creates an unnecessary delay in completing your assessment.

You'll receive email confirmations for each session, complete with appointment details and the option to add them to your calendar. While we consider our assessment sessions as a package, we generally take payment in three equal installments, paid at the time of each appointment.

Comprehensive Assessment

Our assessment process starts with two extended sessions, where we’ll explore in-depth your symptoms, medical history, lifestyle factors, previous and current treatments, and goals. These sessions also include relevant physical assessments and other assessment tools (learn more about how we assess via telehealth in our FAQs). This thorough assessment process helps us to best understand your condition, evaluating a range of factors that may be contributing to your pain or fatigue.

Your Personalised Management Plan

Based on your assessment findings, we collaborate with you to develop a tailored management plan designed to meet your specific needs (usually in the third session of the assessment package). This includes a discussion of potential treatment options and support from us, along with any additional support or referrals you may require from other health professionals. You’ll also receive a report summarizing our findings and recommendations to guide your care moving forward. If needed, we can also provide more specialised reports for NDIS/DSP applications.

Access Ongoing Support

Your treatment plan is just the start of your journey - after your assessment package is complete, we offer follow up consultations to help you progress and implement your plan and work towards your goals. We provide the ongoing support you need, working collaboratively with the rest of your health care team.
